Pharmacodynamics

Contraceptive for oral administration (OK), containing in its composition LIN (progestogen component linestrenol). Entering the body, LIN is transformed into the metabolite of NET (norethisterone), which tends to bind to receptors progesterone in individual bodies, including - myometrium.

Eksluton's contraceptive effect is carried out mainly due to an increase in the viscosity of the mucus of the cervical canal, thereby disrupting the progress of sperm, peristalsis of the fallopian tubes, and the sensitivity of the endometrium to oocytes. As a result of this, almost 70% of women do not ovulate, and as evidence of an increase in progesterone levels and the absence of a peak of growth in the middle of the cycle luteinizing hormone (LH). Taking the drug does not have a clinically pronounced effect on metabolic processes and hemostasis.

Pharmacokinetics

Linestrinol, which is the main active ingredient of the drug, is rapidly absorbed in the digestive tract and is transformed into the metabolite of norethisterone. Bioavailability is 64%. After taking the drug, the maximum levels of Linestrinol in the blood are reached after 2-4 hours. NET binds to a high degree with blood proteins (about 96%), mainly with albumin (61%) and less - with globulin (35%). The metabolites of the drug are allocated mainly in the form of sulfates and glucuronides through the kidneys and intestines. The elimination half-life is about 15 hours.

Exluton, instructions for use (Method and dosage)

Eksluton tablets are taken orally daily for 28 days, one tablet, washed down with water. It is advisable to take the drug at the same time.

Reception of Exluton, in cases of non-use of hormonal contraceptives during the last month, must be started at the beginning of the menstrual cycle (from the first day). If the first day of the cycle is missed, then the tablets can be taken from 2-5 days, but in this case, it is necessary to use the additional barrier method of contraception during the first weeks of the cycle.

If previously contraception was carried out using a combined hormonal contraceptive, then the drug should be started the day after the end of taking the active COC tablet. Methods of additional contraception in this case are not applied.

When switching from taking other drugs based on a progestogen (mini drank, implants, IUDs) Excluton can be started any day immediately after removal of the implant, on the day of the new injection. At the same time, additional protection methods are not used.

With an abortion at the beginning of pregnancy (1st trimester), taking the drug must begin immediately. There is no need for an additional method of contraception.

After an abortion in a later period (2nd trimester) or childbirth, the drug should be started on the 21-28 day abortion or childbirth field. If this period is missed, then you should use the additional barrier method of contraception during the first week of taking the drug. But, in the case of having sexual intercourse after an abortion or childbirth, it is necessary to exclude pregnancy before starting Excluton. It’s better to wait until the menstruation begins.

If you miss the time for taking the tablets (less than 3 hours), the missed dose should be taken as soon as possible, and the next dose at the usual time. If the skip time is more than 3 hours, the barrier method should be used over the next 7 days. If the drug is missed during the first week of use, and before skipping tablets for a week there was sexual contact, the possibility of pregnancy should be taken into account. In case of disorders of the gastrointestinal tract, the process of absorption of tablets in the gastrointestinal tract may be impaired and additional contraception methods must be used.

Overdose

Since the toxicity of the drug is very low, even in cases of taking several tablets at the same time, severe symptoms of poisoning are not expected. Most often noted nausea, vomiting, spotting from the vagina.

Interaction

Exluton, when interacting with other drugs, can cause a violation of contraceptive protection or lead to bleeding. It was found that with the interaction of the drug with barbiturates, primidone, hydantoins, rifampicin, griseofulvin, oxcarbazepine, rifabutin, felbamate, topiramate, ritonavir and preparations containing St. John's wort, women need to use the temporary barrier method of contraception in addition to Exluton tablets or switch to another method of contraception. Moreover, the barrier method must be used not only during the period of use of these drugs, but also for 28 days after the end of their intake.

Caution should be exercised in cases of taking activated carbon preparations, since absorption decreases, and contraceptive effectiveness may decrease.
